Habitat Health, established in 2023, provides integrated health and social care services to older adults on Medicare or Medicaid who wish to remain living independently in their homes and communities. Operating through the Program of All-Inclusive Care for the Elderly (PACE), a joint Medicare and Medicaid initiative, Habitat Health offers comprehensive services including medical care, social services, transportation, personal care, physical therapy, nutrition support, and pharmacy coverage. Their dedicated care teams deliver personalized support both in their centers and in participants’ homes, aiming to enhance the quality of life for seniors. Currently, Habitat Health operates a center in Sacramento, California, with plans for additional locations within the state. The organization collaborates with leading healthcare partners, including Kaiser Permanente, to integrate top-quality clinical teams into their all-inclusive care program.
