According to Kandu Health, "In the United States, 800,000 people have a stroke each year. Historically, most investment in stroke care has focused on acute intervention." But what about after acute intervention? This is the gap Kandu Health is trying to fill to improve long term outcomes for stroke patients, combining care navigators, educational resources, and digital tools to expand the support stroke patients receive longitudinally and improve long term outcomes for patients. And, it appears to be working - they impressively claim that >80% of stroke survivors that have used the Kandu Health program are living independently. (August 2024)
