Role Overview:
The Provider Partnerships Manager will play a critical role in advancing Brave Health’s mission by establishing and nurturing relationships with healthcare providers e.g. practices, and ultimately increasing access to patient referrals from those providers. Here's an alarming stat about mental health readmission rates from a 2022 study from the International Journal of Mental Health Nursing: Unplanned hospital readmission rate is up to 43% in mental health settings. The average readmission rate across general hospital settings? 14%, according to a 2018 study from the Agency for Healthcare Research and Quality. That's a 3:1 higher occurrence of readmission for mental health patients than the general hospital population.
