Job Description
The Manager, Connected Care Care Program for the Interdisciplinary team is responsible for providing direct supervision of the overall daily and long term operations and performance of a diverse team of healthcare professionals, including pharmacists, social workers, dietitians, and psychologists. You will drive team performance in delivering high-quality, patient-centered care and case management services, addressing the medical, psychosocial, nutritional, and mental health needs of patients in a virtual environment. This role involves day to day operational oversight, resource allocation, and ensuring compliance with organizational policies and regulations., reporting directly to the Clinical Director, Care & Case Management and Utilization Management. $138380.0-$257388.0 per-year-salary

- Leadership and Oversight:
- Provide overall leadership and operational direction for the interdisciplinary team.
- Ensure implementation of strategic and process changes designed to achieve outlined goals/objectives.
- Oversee development of department specific goals and objectives, ensuring alignment with Care Management strategies and Included Health's vision, mission, and values.
