Included Health is looking for a compassionate and dedicated Physician generalist who is fellowship trained in Palliative Care to join our Care & Case Management (CCM) Program, where you will play a vital role in supporting members throughout their healthcare journey. Reporting to the Clinical Manager and as part of a multidisciplinary team—including Nurse Care Managers, Dietitians, Pharmacists, Social Workers, and other healthcare professionals—you will provide holistic, patient-centered care through remote consultations and case management. You will develop personalized care plans, offer expert clinical guidance, and empower members to make informed decisions about their health, including managing complex conditions like cancer throughout the care journey. You will collaborate with local treating teams, coordinate resources, and ensure seamless care transitions to improve health outcomes.
