Description Physical Therapy Assistant-Rehab-Per Diem-Day-Mount Sinai Queens Provides treatment according to a plan developed by and under the supervision of a licensed physical therapist, maintains communication with the physical therapist regarding patient plan of care, documents appropriately and maintains their own statistics. Responsibilities Implements treatment programs to assigned patients, based on the treatment plan established by the physical therapist. Assumes responsibility for all documentation requirements in a timely manner. Provides feedback to the physical therapist on a regular basis, indicating the patient's progress in treatment. Coordinates with other disciplines as needed. Pro-actively alerts physical therapist regarding changes in patient condition. Recommends appropriate durable medical equipment in coordination with appropriate team members. Participates in discharge planning, makes recommendations in discussion with physical therapist for continued physical therapy. Attends all appropriate conferences, rounds, in-services and meetings.
