Description

Description The Home Infusion Pharmacist is responsible for facilitating full cycle pharmaceutical care specific to homecare patients, including but not limited to: medication review and management, pump and supply management, prescription fulfillment, coordination with intake and nursing agency teams, and delivery of the drug and supplies to the patient’s home. Responsibilities 1. Participates in patient evaluation and chart review for acceptance, product selection, device selection and supply management. 2. Initiates infusion therapies by assessing the patient, developing a patient care plan and drug monitoring regimen and monitoring the patient according to the care plan throughout the course of therapy. 3. Assists with the referral process including but not limited to: provision of medical information to patients and care givers relative to home infusion therapy; assistance in the teaching of patients prior to discharge to home; and coordination of deliveries with nursing and support services. 4.
