Description Under the direction of the clinic manager, or designee, the counselor shall assist in the personal, social and vocational rehabilitation of a patient being treated for substance abuse by providing supportive services either directly through individual and/or group counseling or by referral to outside agencies. Responsibilities 1. Screen and interview new patients, review records, and confer with other professionals to evaluate individuals opiate/substance use history, mental and physical condition and to determine their suitability for participation in treatment 2. Conduct chemical dependency program orientation sessions. 3. Counsel patients, individually or in group sessions, to assist in overcoming dependencies, adjusting to life, and working toward treatment goals. 4. Review and evaluate patients’ progress in relation to measurable goals described in treatment care plans. 5. Facilitate patient groups and documents the sessions in accordance with APG guidelines. 6.
