Description The Transplant Coordinator is a Registered Professional Nurse, Nurse Practitioner or Physician Assistant with specific knowledge and skills who participates in the coordination of clinical programs and services for designated patient populations within the Transplantation Institute. Responsibilities Responsible For Development, implementation, coordination and evaluation of clinical programs and services for designated patient populations. Principal Duties and Responsibilities Clinical Practice 1. Coordinates the multidisciplinary screening and evaluation of patients seen in the transplant practice at MSH and approved satellites. The multidisciplinary team includes the transplant surgeons, hepatologists, nephrologists, social work, nutrition, pharmacy and finance. Coordinators work with their assistants to facilitate other medical specialty visits as needed. 2. Ensures all necessary lab tests are performed on potential transplant candidates and that results are updated as required (including bloodwork sent to Rogosin for tissue typing). 3.
