Description Job Title: Care Coordinator - Psychiatry Administration RV - BI - Full Time - Day The Care Coordinator is responsible for coordinating care and guidance to medically complicated, high-risk patients who are managing end-of-life care. Develops effective interpersonal relationships with patients and their families, works collaboratively as an integral member of the interdisciplinary care team to develop and coordinate to ensure that patients receive appropriate clinical and social services. Utilizes internal and community resources Responsibilities Works with the interdisciplinary care team to develop a plan of care that addresses all clinical, social, and other needs that patients who have been diagnosed with ALS may need to ensure the highest quality of life for as long as possible. Collaborates with and coordinates all members of the care team and supports the patient and the patients family members.
