Description To be cognizant of the philosophy, standards, objectives and policies of the department and the hospital. Responsibilities A. Clinical/Technical/Service Works with program psychiatrist in assessing medical/psychiatric needs of program patients. Provides linkage and liaison with primary care provider. Provides health evaluation and follow-up to referred patients, management and administration of medication under supervision of MD. Evaluates routine urgent health problems and facilitate, coordinate referrals, evaluate laboratory reports. Provides health mental education, treatment, rehabilitation and support services to referred patients. Services may be provided in the community as needed. Collaborates with program nurse and team members to develop and implement treatment plans involving multiple modalities. Participates in team meetings/rounds. Documents medical record(s) in a timely and accurate manner. B. Organizational/Managerial Participates in the department’s performance improvement activities. Maintains patient/employee confidentiality in the management of information. Observes the Health Care System’s compliance policies. C.
