Description The Stroke Center Coordinator (SCC) is an individual responsible for the coordination of systems and services required to maintain an organized, interdisciplinary, patient centered Joint Commission certified and New York State Department of Health (NYSDOH) designated Stroke Center. Responsibilities include a variety of clinical and administrative functions dedicated to ensure continuity of care, patient satisfaction and compliance with nationally recognized, NYSDOH, The Joint Commission (TJC), and MSHS Standards; as well as an evaluation and improvement of the systems designed to evaluate the effectiveness and quality of comprehensive stroke care delivered. Responsibilities Coordinating all aspects of quality assurance activities of the Stroke Center, together with the identification of patients for inclusion in the hospital stroke center database, data abstraction, collection and the demonstration of performance improvement required by TJC and NYSDOH to maintain their Stroke Center designations.
