Description Outpatient Nutritionist-Mount Sinai Hospital Under the direction of Clinical Nutrition leadership (Senior Director, Director, Clinical Nutrition Manager), provides quality nutrition care by working collaboratively with the multidisciplinary health team within their designated practice area. Provides nutrition counseling and education to patients and family members. The Outpatient Nutritionist provides medical nutrition therapy for all patients in accordance with department and hospital philosophies, standards, objectives, and policies and procedures. Responsibilities Evaluates the nutritional status of the patients by reviewing pertinent information obtained from the electronic medical record and the interdisciplinary healthcare team. Applies knowledge of nutrition screening and assessment to provide medical nutrition therapy by developing and implementing individualized nutrition care plans for general and practice specific patient populations. Provides individualized counseling to patients and caregivers, including management of food-drug interactions, therapeutic diets, and supplements. Conducts nutrition-focused physical exams, as indicated, to determine risk and/or degree of malnutrition. Documents all encounters in the electronic health record per Clinical Nutrition policy. ...
