Description The Certified Clinically Advanced Nutrition Coordinator (CCANC) is responsible for activities that support patient focused care within their clinical specialty area and adheres to department and hospital philosophies, standards, objectives and policies and procedures. The CCANC is an expert in their specialty and acts a resource to both the medical and clinical nutrition staff. The CCANC identifies and collects performance improvement data and coordinates activities for the dietetic interns. This individual assists with administrative duties and provides coverage for the Director of Clinical Nutrition in their absence. Responsibilities 1. Evaluates the nutritional status of patients by reviewing pertinent information obtained from the electronic medical record and the interdisciplinary healthcare team. Applies knowledge of nutrition screening and assessment to specific patient populations to determine their nutritional needs. Conducts nutrition-focused physical exams. 2. Possesses population-specific skill set and advanced credential in clinical nutrition assessment. 3. Documents pertinent findings in the medical record per policy. 4.
