**What is the role?**The Nurse Care Manager (NCM) reports to the Health Services leader and provides oversight, guidance and support for family care plans developed in collaboration with Nest Health clinicians and interdisciplinary team members. The NCM maintains a deep understanding of the patient panel and helps synthesize and prioritize family and patient needs and action plans based on family goals, acuity, quality gaps, and social determinants of health. In collaboration with other members of the healthcare team, the NCM is responsible for organizing, coordinating, and providing care coordination and care management services to patients and families.
