Pharmacist- Per Diem

In this role, you will review and interpret medication orders by physicians to other authorized healthcare providers. Verifies orders for completeness, accuracy, and appropriateness in accordance with pharmaceutical care standards and prescribed policies and procedures.

Reviews patient’s profiles and problems, i.e., Drug interactions, drug allergies, therapeutic duplication, contraindications, drug-drug incompatibilities, dosing, and adverse drug reactions relating to the new order or order changes. Maintains effective communication with physicians and nurses to ensure safe and appropriate use of drugs.
