Social Worker in Integrated Behavioral Health - Bilingual Spanish - ACN Columbia Days

Provide short-term mental health services to adult patients and families across our Ambulatory Care Network Primary Care Clinics at the Columbia Campus.  The social worker partners collaboratively with the care team to meet the psycho social and behavioral health needs of patients identified through pre-visit screening and in the primary care visit.   The social worker will work with adult patients and families.

The social worker will triage patients; complete clinical assessments; deliver evidence-based psychosocial and behavioral health education and interventions to patients, families, staff, and medical providers; recommend treatment and referral options to meet the stated behavioral health and psychosocial needs of patients; and connect them with needed longer-term behavioral health services as needed.  The work includes crisis intervention, real-time safety screening for suicidality and intervening as clinically indicated.
