Patient Care Director - Hospital at Home - Full time Days
Hospital at Home is a unique and innovative program at NYP that provides inpatient hospital level care at the patient’s home. The program combines daily in-home visits, virtual visits, and 24/7 remote patient monitoring to give eligible patients an opportunity to stay in the comfort of their own home while receiving their hospital care. This program will be designed and delivered by NYP providers and clinical staff, allowing NYP's best-in-class care across all measures of quality, service, cost, and innovation to be continued in the home setting. Hospital at Home will provide strategic value to NYP by alleviating capacity constraints, improving patient experience, reducing readmission rates, and driving cost savings to the organization. Since 2020, CMS has authorized more than 320 hospitals across 133 health systems in 37 states to offer acute hospital care at home, and those numbers are climbing.
