The Care Coordinator works collaboratively with the care team to provide ongoing support and communication to chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD) patients. Did you know Medicare expenditures for Chronic Kidney Disease (CKD) was $85Bn in 2020? I had to do a double take as well - this represents nearly a quarter of all Medicare expenditures, yet counts only 14% of Medicare beneficiaries over 66 years old. This is a massive hole in the Medicare budget - and a massive challenge for patients with CKD both financially and from a care perspective. Enter Strive Health, which is looking to disrupt the market using value-based care principles to more accurately track and treat patients suffering from CKD. They raised $166mn from their Series C raise alone - a massive amount for a company, even more so from a single funding round. Big funding for a big problem for patients, providers, and payers.
