We provide flexible, specialized care for patients and caregivers living with neurodegenerative diseases. As a Patient Engagement Specialist, you will be responsible for locating and connecting with patients who would benefit from our care model and engaging with them to understand and address their needs. Your interactions will be the patient's first encounter with our organization, initiating a relationship that will later transition to a Care Coordinator for long-term support. Until this transition occurs, you will serve as the patient’s primary contact, ready to assist with support, education, or simply providing a listening ear. You will work closely with our care team to ensure that patients and caregivers receive the comprehensive support they deserve from the very beginning. Location: This position is fully remote, with all necessary technology provided to work from home.
