About the RoleThe Care Navigator is a direct support to people affected by neurodegenerative diseases. This person oversees the relationship, enrollment, documentation and care experience of patients through the Synapticure program as well as the development of the Care Coordination team workflows and processes in collaboration with your peers and other leaders. Applicants should be passionate about the power of involving patient voices in their care experiences and outcomes, and should thrive on direct patient support, particularly for vulnerable populations.
