The Social Services Coordinator will be responsible for serving as a primary liaison between Centrum Health and The Department of Children and Families Office Economic Self-Sufficiency (ESS) ESS Partner Network. 

DUTIES & RESPONSIBILITIES
•    Assist in completing benefit applications for public assistance programs. 
•    Links community residents to appropriate public assistance programs from both public and private funding. 
•    Schedule appointments for follow-up and provides assistance and/or referrals for essential support services such as, but not limited to, housing, financial aid, food stamps, Medicaid, emergency food, WIC, clothing, transportation and day care.
•    Participate in county, city and other coalitions to promote the available social services programs and identify collaboration opportunities. 
•    Provide community presentations and attend community meetings and outreach events to promote programs. 
•    Process Medicaid renewal for eligible plan members.  
•    Provide both remote and on-site assistance.  
•    Maintains patient confidentiality and complies with HIPAA guidelines.  ...
