The reality: huge communities of seniors need access to medical care. Enter Pine Park Health, which is blending primary care, in-home care, and geriatric care for senior living communities to give patients greater access to care from the comfort of their homes. They've grown to over 100 community partners across the west coast, and are backed by notable investors including Google and YCombinator - and the growth of the over-65 population doesn't appear to be slowing down any time soon.
