Pine Park Health is a pioneering primary care company that delivers high-quality mobile healthcare services to seniors residing in assisted living communities. Our innovative care model, centered around nurse practitioners, aims to provide accessible and compassionate care to our elderly population. As we transition from a fee-for-service model to value-based care, we seek a dedicated and experienced Vice President of Clinical Education to lead our educational initiatives and support our team through this significant change.
The Opportunity
As a Provider at Pine Park Health, you will be at the forefront of delivering personalized, in-person primary care to a high-acuity geriatric population residing in senior living communities. You will serve as the cornerstone in a comprehensive healthcare model that focuses on clinical excellence, meaningful community engagement, and proactive patient education. You'll be responsible for a spectrum of care—from preventive and routine care to urgent clinical interventions—all while fostering strong relationships within the community. The reality: huge communities of seniors need access to medical care. Enter Pine Park Health, which is blending primary care, in-home care, and geriatric care for senior living communities to give patients greater access to care from the comfort of their homes. They've grown to over 100 community partners across the west coast, and are backed by notable investors including Google and YCombinator - and the growth of the over-65 population doesn't appear to be slowing down any time soon.
