We empower seniors to get healthy, stay healthy, and lead lives they love through a care model designed with everyone in mind — patients, families, community staff, providers, and payers.
Our approach brings healthcare directly to seniors' homes, offering prevention, screening, chronic condition management, lab work, and diagnostic testing in the comfort of their apartments. With weekly community visits, our teams provide consistent care while collaborating closely with facility staff to address comprehensive health needs. We've eliminated unnecessary barriers to urgent care through same/next-day appointments, helping seniors avoid emergency rooms and hospitalizations where they risk exposure to additional health concerns.

Here's a mindblowing stat about aging in the US: in 2020, 1 in 6 people were over 65. In 1920, this was less than 1 in 20. Here's another one: from 2010-2020 alone, the number increased by 15.5 million. With this huge spike in the aging population, senior living communities have grown massively in popularity. The reality: huge communities of seniors need access to medical care. Enter Pine Park Health, which is blending primary care, in-home care, and geriatric care for senior living communities to give patients greater access to care from the comfort of their homes.
